I offer a free “mini-consultation” phone call for new clients. If we decide to work together my rate is $200 per session. Session times may be extended by request as indicated if and when appropriate. Payment must be made in full at the time of each session unless we have come to another agreement.
Sliding Scale/Reduced Fee:
I do have a certain number of sliding scale spots available for clients with financial needs. Please contact me at 917-512-5692 for more information about my rates and to find out if I currently have a sliding scale spot available.
Insurance:
Services may be covered in full or in part by your health insurance or employee benefit plan. With PPO coverage, I prepare a Superbill for you to submit to your heath insurance provider who may then reimburse you according to your plan specifications. Currently I do not bill insurance companies directly. However, I can provide you with a receipt for services that you submit to your insurance company for out-of-network reimbursement. I can provide you with a monthly receipt, should you choose to seek reimbursement from your insurance company. It is up to you to verify coverage.
Please check your coverage carefully, contact your insurance company directly and ask the following questions to find out:
Do I have mental health insurance benefits?
What is my deductible and has it been met?
Does my plan provide reimbursement for out-of-network mental health care? If so, is there a separate deductible?
How many sessions per year does my health insurance cover?
Is there a limit to the number of sessions that will be reimbursed per year?
What is the coverage amount per therapy session?
Do I need pre-authorization for out-of-network services?
Is approval required from my primary care physician?
Appointments and Cancellations:
Your appointment time is reserved for you. If you need to cancel or reschedule an appointment, a minimum of 24 hours notice is required in order to avoid being charged. You are responsible to pay for your session in full if a cancellation request is received less than 24 hours in advance.
